File a Report

We receive and review all reports of Board of Governor’s Rule 1.6 and Rule 1.7 violations. If you believe that a Title IX violation has occurred, or have suspected or observed child abuse/neglect, you are encouraged to use one of the forms below.

Student Report Form

If the Accused/Respondent (“Respondent” is the term used for a person accused of violating a University policy) is a WVU Student, or if the Accused/Respondent is Non-WVU but the Victim/Complainant is a Student, please use this form to file a report.

Employee Report Form

If the Accused/Respondent (“Respondent” is the term used for a person accused of violating a University policy) is a WVU Employee, or if the Accused/Respondent is Non-WVU but the Victim/Complainant is a WVU Employee, please use this form to file a report.

Report Child Abuse/Neglect

This form is used to file a report regarding any type of child abuse or neglect suspected or observed by the University Community. This was priorly found at the “Report Abuse” button found at the top of this page.

Purpose of These Forms

These forms may be filled out by the alleged victim/survivor (complainant) of conduct prohibited by Rule 1.6, mandatory reporters as outlined Rule 1.6 (including WVU faculty, staff, and/or employees), or any other person who witnesses or has knowledge of such alleged conduct.

If you wish to remain anonymous, you may seek services via our 24/7 Title IX Anonymous Hotline at (304) 906-9930 or file a report with this request. However, please note that the ability to investigate an anonymous report is severely limited.

For the confidentiality of all parties, please understand that we may not follow up with you unless you are directly involved in this case, but we will always follow up with the people who were directly affected by the behavior you described in this report form. For example, we would not typically reach out to a professor who filed a report about something they heard secondhand from a student unless we have follow-up questions. Instead, we would contact the student directly to make sure they were OK. We hope you understand that we hold confidentiality in the highest regard.

Important Note for Victims/Survivors (Complainant)

If you are a Victim/Survivor of sexual assault, sexual violence, domestic violence, dating violence, or stalking and do not feel safe, please call 911 immediately OR contact the University Police Department at 304-293-3136.

  • WELLWVU Carruth Center for Counseling and Psychological Services
    • Counselors and licensed psychologists offer free, confidential counseling and advocacy specifically for WVU Students.
    • Services are available 24/7 at (304) 293-4431. The Carruth Center is located in the Student Health Building (STH) on the Evansdale campus across from the Student Rec Center. Please enter via the doors by the Mountaineer Pharmacy. There are two sides of the building; we are on the Student Health Services side. The building is a short walk from the Towers PRT station.  There is also parking out front if you choose to drive. Reporting Procedure, Requirements and Exemptions

Anyone with knowledge of discrimination, harassment, sexual harassment, sexual misconduct, domestic misconduct, stalking, retaliation, and prohibited relationships is encouraged to fill out this form. If you are a West Virginia University employee and have such knowledge, you must fill out this form unless you have been explicitly told that you are a professional or pastoral counselor who is exempt from such disclosure. If you are making this report on behalf of someone else, you should let that person know that you must report what you have learned, but that confidential counseling is available at WELLWVU’s Carruth Center for Counseling and Psychological Services and that someone will follow up to provide resources and reporting options.
